CDR Program Liaison

Under general supervision, this position is responsible for ensuring seamless care for patients by acting as liaison between patients, physicians and hospital. They are also responsible for understanding and practicing the skills necessary to facilitate patient's initial contact with Center for Disc Replacement by scheduling new and follow-up patient visits via telephone or email that may include assisting with travel and hotel accommodations if needed.
  • Promote Center for Disc Replacement Program and cultivate customer experience to help establish long-term relationship with consumer
  • Coordinate with patient and referring physician offices to act as a navigator to assist with any needs to obtain any required documentation, necessary diagnostic testing, medical records or missing information for patient appointment
  • Educate patients inquiring about Center for Disc Replacement regrading spine health, spinal treatment options, artificial disc replacement, treatment pathway, medical travel and insurance.
  • Work closely with the patient to assist with any needs pertaining to accommodation and other services as required
  • Follow thru on patient outcomes from inception to completion and managing their expectations
  • Receive and manage all phone and internet inquires related to Center for Disc Replacement
  • Triage phone calls and inquires to ensure patients are scheduled with appropriate physician
  • Document referrals and leads in proper system
  • Schedules patient appointments
  • Scanning and Indexing of clinic paperwork into patient chart
  • Assist with outreach, marketing or other events when needed
  • Performs various duties and assist with program & research projects as needed and discussed with Director
  • Treats customers and co-workers with our services standards at all times
  • Attends and participates in department meetings and training classes
  • Maintain and promote HIPAA compliance in all areas
  • Others duties as assigned

Physical Requirements

While performing the duties of this job, the employee is regulator required to sit. The employee is frequently required to use bands, fingers to handle or feel, occasionally walk or stand and infrequently carry boxes/supplies of 10 lbs or less.

Minimum Requirements

  • High School Diploma or GED, prefer associate degree
  • Two or more years of successful experience in medical field, phone sales or lead generation experience
  • Demonstrated and proven customer service and results
