To fulfill the intent of Queen Emma and King Kamehameha IV to provide in perpetuity quality health care services to improve the well-being of Native Hawaiians and all the people of Hawaii.

Scope of Service

We provide intermittent quality care for homebound patients, of all ages, by offering a network of services to meet the emotional and physical needs of both patients and families.   We provide Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, as well as Certified Home Health Aide care, and Medical Social Worker services.  Our service area includes Hilo, Hamakua, North and South Kohala, North Kona, Keaau and Volcano.


  Under general direction of Director or Manager, responsible for providing occupational therapy  services, including but not limited to screening; examination/re-examination; evaluation; diagnosis; prognosis; plan of care; interventions; outcomes assessment; education, practice management; professionalism; consultation; evidence based practice; cultural competency; and promotion of health, wellness and prevention.
  Depending upon area of assignment, provides a variety of  occupational therapy services to assist patients regain physical or mental functioning or adjust to disabilities by implementing programs in activities of daily living.
  Responsible for the assessment, treatment, or care of all ages.  Responsible for individualized treatment plan that reflects the developmental cognitive, physical, emotional, and chronological maturation process and range of treatment needs for each patient.

II. TYPICAL PHYSICAL DEMANDS:  All essential, designated by frequency.
  Essential: seeing, hearing, speaking, standing, walking, finger dexterity, lifting usual weight of 15 pounds up to maximum 35 pounds, pushing/pulling up to maximum 50 pounds of force, carrying usual weight of 15 pounds.
  Continuous: reaching below shoulder.
  Frequent:  sitting, stooping/bending, reaching above/below shoulder, squatting, twisting body, repetitive arm/hand motions.
  Occasional:  kneeling, crawling, walking on uneven ground, static gripping of an object for prolonged periods of time, frequent gripping of an object. .
  Operates clinical equipment of the KHH unit, computer, FAX machine, copier, multi-line telephone, printer, cell phone, lap top/tablet.

  Not subjected to adverse environmental conditions. 
  Occasional exposure to safety hazards, diseases, or contamination.
  Exposure to body fluids and wastes; hazards from electrical
  Works with mechanical/power equipment; subjected to injury from moving parts of equipment.
  May perform emergency care.
  Subjected to many interruptions and variables throughout the day.
  Required Personal Protective Equipment:  Eye protection, gown, mask, non-sterile or sterile medical gloves, booties and bonnet.

  Current Hawaii State license as an Occupational Therapist.
  Current BLS/HCP certification.

  Two (2) years experience as an Occupational Therapist.  Depending upon area of assignment, Home Health experience may be preferred.
  Experience to demonstrate the following:
  o  Knowledge and skills to provide care appropriate to age of patients served.
  o  Knowledge of state and federal guidelines/regulations.
  o  Ability to communicate effectively, verbally and in writing.
  o  Strong organization and planning skills:  anticipate tasks/prioritize assignments;  manage multiple projects, varying schedule and clinical load.
